The 5×5 risk matrix is the foundation of risk scoring across your entire SMS. Likelihood × Severity produces a risk score that Hubfleet uses to colour-code risks, trigger escalations and rank items in your Risk Register. Hubfleet ships with sensible defaults — most operations only need to relabel a few rows to match the language they already use. This is Step 1 B of your Safety Management System (Preview) setup — complete it after your Setting Up Your Safety Org Chart and before your Setting Up Escalation Rules.

How the matrix drives the SMS

Every hazard, incident, NCR and fault raised in Hubfleet is scored on the same 5×5 matrix. Each axis is rated 1–5 — multiplying them gives a 1–25 risk score that Hubfleet maps to one of five colour-coded bands. The resulting band determines:

• The colour the item appears in across the Risk Register, hazard lists and incident dashboards
• Whether the item auto-escalates to your Escalation Contact, via the rules you set up in Step 1 C
• How it's ranked and prioritised alongside everything else

Configuring the matrix consistently is what lets you compare a Fatigue NCR against a Maintenance fault against a hazard report on the same scale.

The score-to-band mapping is fixed in Hubfleet — you're customising the labels on the rows, columns and bands, not the underlying score ranges.

What you'll configure

• Likelihood labels — five rows from least likely to most likely (defaults: Very Low → Very High)
• Severity labels — five columns from least severe to most severe (defaults: Very Low → Very High)
• Risk Rating bands — the five colour-coded outcome bands (defaults: Very Low → Very High; a common rename is Very High → Extreme)

Hubfleet ships with defaults for every row, column and band. Customise only the labels that don't match your operation's language.
