The 5×5 risk matrix is the foundation of risk scoring across your entire SMS. Likelihood × Severity produces a risk score that Hubfleet uses to colour-code risks, trigger escalations and rank items in your Risk Register. Hubfleet ships with sensible defaults — most operations only need to relabel a few rows to match the language they already use. This is Step 1 B of your Safety Management System (Preview) setup — complete it after your Setting Up Your Safety Org Chart and before your Setting Up Escalation Rules.
Every hazard, incident, NCR and fault raised in Hubfleet is scored on the same 5×5 matrix. Each axis is rated 1–5 — multiplying them gives a 1–25 risk score that Hubfleet maps to one of five colour-coded bands. The resulting band determines:
Configuring the matrix consistently is what lets you compare a Fatigue NCR against a Maintenance fault against a hazard report on the same scale.
