This is Step 3 A) of building your SMS — once your Hazard Register, controls and risk processes are set up, drivers can start feeding the system from the field.

Field hazard reporting lets any driver or supervisor raise a hazard directly from the Hubfleet mobile app. Reports include photos, GPS coordinates and timestamps. From there the hazard appears in the Hazard Register on the web portal, where your safety or compliance team picks it up and runs it through the risk management process.

Mobile is for reporting. Web is for managing. Drivers and supervisors can only report a hazard from the mobile app — fill in the form and submit. Everything else (risk rating, controls, actions, status changes, escalation, closure) happens on the web portal and is the safety or compliance manager's job. How field hazard reporting fits into the SMS

A hazard reported from the field connects to several other parts of your SMS:

• The right person picks up the report once it's risk-rated — defined by your Setting Up Your Safety Org Chart
• The hazard lives in the Setting Up Your Hazard Register, where it's risk-rated using the Configuring the 5×5 Risk Matrix and the calibration anchors from ‣
• Setting Up Escalation Rules decides who gets notified and what approvals are required, based on parent item type (Hazard), Safety Category and risk level
• Controls and actions added to the hazard flow into the Actions Register

Overview

• A driver or supervisor opens Hazard Reports in the Hubfleet mobile app and submits a report
• The hazard appears in the Hazard Register on the web portal
• A safety or compliance manager picks it up and runs the full risk management process (risk rating, controls, actions, escalation, closure)
